GENERAL INFORMATION

Gather New Haven offers our Farm-Based Wellness program (FBWP) every summer, a program designed for individuals diagnosed with or at risk for diet-related health conditions such as diabetes mellitus, heart disease, etc. 

The FBWP is an intensive lifestyle intervention summer program that meets for two hours each week for 16 weeks in duration. The program offers sessions in both Spanish and English, on Tuesday and Thursday, respectively. 

Our program materials and curriculum have been adapted from the Center for Disease Control’s Diabetes Prevention Program (DPP), an evidenced-based lifestyle modification program designed to help individuals make lifestyle changes to both prevent diabetes and prevent or treat metabolic syndrome. We work with local providers and health centers that refer their patients to our program.

ELIGIBLE PROGRAM PARTICIPANTS

This program is designed for individuals, age 18 and older who have been diagnosed with pre-diabetes, diabetes, metabolic syndrome, or other chronic diet related diseases.

TO MAKE A REFERRAL

Health care providers can refer via EPIC, using the referral name New Haven Farms.

If you are a health care provider that does not use EPIC, you can refer by getting verbal consent from the patient and emailing Program Manager Peter Zapata (peter@gathernewhaven.org) with the patient name, DOB, phone number and risk factors or diagnosis.

Providers can also tell their patients to text “wellness” or “salud” to 970-00 or call (203) 516-7270 to receive more information!
